Which packet do I need?

Receiving CFS services through your child’s school? Choose the school-based intake packet.

All other clients: Choose the general intake packet. Both packets are available in English and Spanish.

If you are unsure, call Intake at (864) 283-0637, ext. 200.

Use this packet for all clients who are not receiving CFS services through their school.

School-based services

Use this packet for a child receiving CFS services through a participating school.

The school-based packets are larger files and may take a little longer to open.

Completing and returning your packet

You can save or print the PDF using your device’s PDF viewer. Follow the instructions from your CFS team for completing and returning your paperwork before your assessment.
